1. Registration of Death Details
These details will need to be submitted by your licensed funeral director (Basic Country Cremations) on you behalf in order to officially register that a death has occurred with Births Deaths and Marriages, and to obtain an offical death certificate.
2. Application for Cremation Form
This form is a legal requirement under the Cemeteries and Crematoria Act 2003.
